Behavioral Health Services

Offered in Partnership with CREOKS

At 40:31 Rehab & Wellness, PLLC, we believe that meaningful progress happens when we support the whole child - and the family surrounding them. We are excited to expand that support through a partnership with CREOKS Health Services, bringing behavioral health professionals and additional family-centered resources alongside the our outpatient pediatric services like Occupational Therapy and Speech Therapy services provided by our team.

Through this partnership, CREOKS Licensed Professional Counselors (LPCs) and Licensed Clinical Social Workers (LCSWs) can provide behavioral health, counseling, family support, advocacy, and resource-navigation services for pediatric clients and their families.

This collaboration allows our therapy team and CREOKS behavioral health professionals to address different - but often interconnected - factors influencing a child's participation at home, school, and in the community.

Supporting the Whole Child

Children do not experience sensory, developmental, emotional, behavioral, and environmental challenges in isolation. A child struggling with regulation may also be experiencing anxiety. A child with ADHD may need support with executive functioning as well as emotional coping skills. A child with a history of trauma may demonstrate behaviors that affect relationships, learning, sensory regulation, and participation in everyday routines.

Through our partnership with
CREOKS, families can access behavioral health support for needs including:


  • Trauma and adverse childhood experiences, including abuse, neglect, grief, family disruption, foster care experiences, and significant life transitions.
  • Autism Spectrum Disorder (ASD) and associated emotional, social, behavioral, and family-support needs.
  • ADHD, including emotional regulation, impulsivity, executive functioning, behavior, family stress, and coping skills.
  • Anxiety, including fears, avoidance, difficulty with transitions, school-related anxiety, and challenges coping with uncertainty.
  • Emotional regulation and behavioral concerns.
  • Peer relationships, social participation, and interpersonal challenges.
  • School-related stress, adjustment, and behavioral health needs.
  • Family circumstances that may be affecting a child's emotional well-being or participation in everyday activities.

Counseling & Behavioral Health Support

CREOKS behavioral health professionals can provide counseling and therapeutic support that complements the developmental services children may already be receiving through 40:31 Rehab & Wellness.

When appropriate and with appropriate authorization, our teams can collaborate to help families establish consistent goals and strategies across disciplines.

For example, a child receiving
Occupational Therapy for sensory processing, self-regulation, executive functioning, or daily living skills may simultaneously receive behavioral health services through CREOKS to address anxiety, trauma, emotional coping, or family dynamics.

Similarly, a child receiving
Speech Therapy for communication or social-pragmatic needs may benefit from behavioral health support addressing social anxiety, relationships, emotional expression, or other factors affecting successful participation.

The result is a more comprehensive approach in which developmental, functional, emotional, and environmental needs can all be considered.

Family Resources, Advocacy & Care Coordination

Sometimes the greatest barrier facing a child has very little to do with what happens inside a therapy session.

Families may be navigating housing instability, food insecurity, school concerns, disability applications, financial stress, foster care systems, healthcare access, or difficulty understanding which community resources are available to them.

Through our partnership with CREOKS, families may also receive assistance identifying and navigating resources related to:


  • Food and nutrition assistance
  • Housing and community resources
  • School-based services and educational advocacy
  • IEP and 504 supports
  • Disability resources, including assistance navigating application and approval processes
  • Community behavioral health and healthcare resources
  • Foster care and family-support programs
  • Additional community services based upon the individual needs of the child and family


The goal is to help families move beyond simply a single diagnosis, symptom, or therapy discipline.

At 40:31 Rehab & Wellness, our Occupational and Speech Therapy professionals can continue addressing the developmental and functional skills children need to participate more successfully in everyday life, while CREOKS behavioral health professionals can address emotional and behavioral health needs and help families navigate resources and supports outside of the therapy clinic.

Together, these services can create a more connected system of support surrounding the child and family.
